Steps to Getting Insurance to Pay for a Cubby Bed

1. Have your child's doctor write a prescription for a "Cubby Safety Bed" with medical necessity on it. We had ours written to NuMotion #954-473-9534

2. Reach out to the DME(durable medical equipment). In our case it was NuMotion. I did so to confirm they received the prescription and to fully find out how to go about the process.

3. Have your child's OT or PT write a very descriptive medical necessity letter. In this letter you want to make sure it shows

A. Your child's diagnosis/diagnoses, age and height

B. List ALL the low tech methods you have tried to ensure your child's safety/elopement or ways you've tried keeping them in their current bed(like nightlight, weight-blankets, lavender baths, melatonin, putting in door chimes for exterior doors, etc)

C. Include how your child's sleeping patterns have affected their behavior and overall how it has affected their health.

D. Have your OT include how the special features on the Cubby Bed would benefit your child.

(I feel our OT is why our son's got approved. She went above and beyond in her letter. Our family could never thank her enough for what she did.)

4. Our DME NuMotion sent out someone to do some type of Evaluation with our son's OT. Before submitting it to insurance. You will have to sign/initial some papers as well.
